Just ran across a new feature in Google Docs - Google Forms

In one of the last meetings there was a discussion of if we had an easy form creator. This seems to fit the bill, as it was real fast to create a form, and the responses go in a spreadsheet.
Here is an example of one (based on the pilot project idea):

Setting up the form was fast as templates are provided.
Could also be a useful tool for faculty to collect info, or students to collect data etc. etc. Seems like a workshop in Google Docs for teaching may be useful.
